Young people with long-term conditions have seen their life expectancy improve significantly over the last decades, with more than 85% of children with chronic conditions reaching adulthood and the majority of them requiring an organised transfer to adult care. In an attempt to improve the outcomes of young people transitioning from childrens’ to adult services, ‘Together for Short lives’ - the United Kingdom’s charity for children’s palliative care - decided to support a collaborative project with the Royal College of General Practitioners. This project is centred around the multidisciplinary palliative primary care teams, which are usually already present in most general practices around the United Kingdom. It aims to extract existing knowledge and evidence gathered in pathways and identified through difficulties and issues for young people with life-limiting conditions in primary care and then apply them in pilot projects in practices around Britain.
