Patient Safety Toolkit

Patient safety is the absence of preventable harm to a patient and reduction of risk of unnecessary harm associated with health care to an acceptable minimum. Health care has become more complex, requiring greater use of new technologies, medicines and treatments. As we treat older patients, with more co-morbidities, decision making gets more difficult and the risk of errors and adverse effects rises. These three NUBs will give you more information on patient safety principles and processes, as well as detailed instructions on how to carry out a trigger tool analysis. The resources section gives more information about patient safety, including the opinion of regulatory bodies and templates that practices can use to analyse patient safety or carry out a significant event analysis.
