Patient safety is the absence of preventable harm to a patient and reduction of risk of unnecessary harm associated with health care to an acceptable minimum. Health care has become more complex, requiring greater use of new technologies, medicines and treatments. As we treat older patients, with more co-morbidities, decision making gets more difficult and the risk of errors and adverse effects rises.
